Building Permit:
(New Construction, Modular Homes, Additions, Remodels, and Decks)

The Building Permit Process - A checklist of the forms/plans needed to submit for a building permit.

• Building Permit Application
• Affidavit as to Status of Licensure
• Self Contractor Questionnaire
• Affidavit of Worker’s Compensation Coverage
• License Check and Regulation Sheets:
  • Electrical Contractor
  • Plumbing Contractor
  • Heating Contractor
  • Fuel Piping Contractor
• Plan Specifications (2 Copies)
• Erosion Control Certification

Manufactured Home:
(Single or Double Wide)

Please submit the following forms along with a copy of the Authorization for Wastewater System Construction (septic permit) and well construction from the Appalachian District Health Department:

• Mobile Home Permit Application
• Electrical Contractor License Check and Regulation Sheet
• Erosion Control Certification

Alteration Permit:
(Service change, heat change-out, well service, etc.)

Please submit the following application along with the correct sub-contractor form:

• Alterations Permit Application
• Contractor License Check and Regulation Sheet
  • Electrical Contractor
  • Plumbing Contractor
  • Heating Contractor
  • Fuel Piping Contractor
