Affidavit as to Status of Licensure
Self Contractor Questionnaire
Affidavit of Worker’s Compensation Coverage
Contractor License Check and Regulation Sheets:
Plan Specifications (2 Copies)
Erosion Control Certification

Manufactured Home:
(Single or Double Wide)

Please submit the following forms along with a copy of the Authorization for Wastewater System Construction (septic permit) and well construction permit from the Appalachian District Health Department:

Alteration Permit:
(Service change, heat change-out, well service, etc.)

Please submit the following application along with the correct sub-contractor form:

Alterations Permit Application
Contractor License Check and Regulation Sheet
