Community Alternatives Program

The Project on Aging coordinates with home health agencies which provide the following:
  • Nursing Services
  • Speech, Occupational and Physical Therapy
  • Medical Supplies
  • Durable Medical Equipment
  • Nurse Aide Services
Clients must be at least 18 years of age and certified in the need of ICF or SNF Nursing Home Care, qualified by a physician, and meet Medicaid eligibility guidelines.


Who May Apply
  1. Client must qualify for Nursing Home (SNF or ICF) and need help with personal care.
  2. Must be at least 18 years of age.
  3. Client must be on full Medicaid or apply to qualify for full Medicaid.
  4. Client must have informal support from other persons to cover non-covered hours.
  5. Either the Client or a designated representative may initiate an application.
How To Apply
  1. Call or come by Watauga County Project on Aging, Monday – Friday, 8:00 a.m. – 5:00 p.m.
  2. A CAP Case Manger will counsel them.
  3. If CAP is to be pursued an FL-2 must be completed by the physician.
  4. If not covered by Medicaid they will apply for CAP Medicaid at the Department of Social Services. You should bring your Social Security Card (if currently covered) and the previous month’s bank statement. Also bring the following:
    • Medicare card
    • Birth Certificate or proof of age
    • Letters or forms showing the amount of income from Social Security SSI, VA, retirement, etc.
    • Life and medical insurance policies for applicant
    • Information on ownership of real property and motor vehicles.

CAP Finances
  1. Client must be on or apply to be on full Medicaid (Blue Card).
  2. A Hospice patient can be on CAP only if also covered by Medicare or if Hospice waives the fee.
  3. The client may have a deductible. The Department of Social Services Medicaid Worker determines this.
  4. Qualifying medical expenses may be used toward the deductible, as determined by the Department of Social Services Medicaid Worker.
CAP Benefits
  1. Home visit from a Registered Nurse.
  2. No limit on the number of prescriptions each month and no copay.
  3. Purchase and rental of Durable Medical Equipment and supplies.
  4. In-home Aide Services.
  5. Case Management.
  6. Home Delivered Meals.
  7. Home Mobility Aids.
  8. Telephone Alert.
